How semaglutide produces weight loss

Semaglutide activates GLP-1 receptors throughout the body — in the brain, stomach, and pancreas — producing several complementary effects:

  • In the brain: reduces hunger signals and food cravings, particularly for high-calorie foods
  • In the stomach: slows gastric emptying, creating earlier and more prolonged fullness
  • On metabolism: improves insulin secretion and sensitivity, better regulating blood glucose

Weight loss is dose-dependent and progressive, with most patients reaching maximum weight loss around 60–68 weeks of continued treatment. Most patients require ongoing use to maintain results — discontinuation commonly leads to weight regain.

Why excess skin develops

Significant weight loss stretches skin beyond its natural capacity to retract. Carrying excess weight for extended periods permanently stretches and damages the collagen and elastin fibres providing skin structure — proteins that cannot fully repair themselves once compromised.

Duration of excess weight matters too: 5–10 years causes moderate skin elasticity damage, 10–20 years causes significant damage, and 20+ years often causes severe, sometimes permanent elasticity loss. Age, genetics, sun exposure, smoking history, and prior weight fluctuations all factor in as well.

Body contouring options

For moderate weight loss (25–50 lbs): targeted procedures often give excellent results.

For significant weight loss (50–80+ lbs): more comprehensive procedures, sometimes staged 4–6 months apart, become appropriate.

Facial volume loss (hollowed cheeks, sunken temples) sometimes seen with rapid or significant weight loss often improves naturally over 6–12 months. Persistent concerns can be addressed with dermal fillers or other facial procedures — outside the scope of what Dr. Borsuk offers, but worth discussing with an appropriate provider.

Timing your procedure

Wait 6–12 months of documented weight stability before pursuing body contouring surgery. Continuing semaglutide for weight maintenance is safe and often medically advisable after body contouring — but your weight should be stable, not actively declining, at the time of surgery.

Individual skin quality can differ, and some relaxing of skin can occur over time even after surgery — this usually still leads to comfortable, natural-feeling results. Maintaining stable weight, with or without continued medication, is essential for preserving outcomes.
