Weight Loss & Body Contouring

Duodenal Switch and Excess Skin

Understanding this bariatric procedure, why excess skin often follows it, and when body contouring surgery becomes an option.

Duodenal switch is one of the most effective bariatric procedures available. This complex, two-part surgery combines restrictive and malabsorptive techniques — first reducing stomach capacity through sleeve gastrectomy, then rerouting the small intestine to limit calorie absorption. Patients typically lose 60–80% of their excess body weight within 18–24 months following surgery.

This transformation represents an enormous achievement requiring real commitment to dietary protocols, vitamin supplementation, and lifestyle change. But losing 100 or more pounds in a relatively short timeframe often leaves substantial excess skin behind. This is a normal biological outcome, not something within your control to prevent — skin simply cannot keep pace with such dramatic, rapid weight reduction.

Important: Dr. Ronnie Borsuk does not perform duodenal switch or any bariatric surgery. He specializes exclusively in excess skin removal and body contouring for patients who have already completed their weight-loss journey — through bariatric procedures, GLP-1 medications, or lifestyle change.

Why excess skin develops

Carrying significant excess weight for years causes skin to stretch continuously. Over time, the collagen and elastin fibres responsible for skin elasticity become permanently stretched and weakened, and cannot repair themselves once stretched beyond their natural capacity. When rapid weight loss follows, skin lacks the elasticity to retract and conform to your new frame.

Duration of excess weight

10–20+ years causes more extensive skin damage than shorter durations.

Age at weight loss

Younger skin retains more elasticity and collagen production.

Genetics

Family history significantly influences skin quality and elasticity.

Skin quality factors

Sun exposure, smoking history, and hydration all affect skin health.

Rate of weight loss

Rapid loss gives skin less time to adapt gradually.

Total weight lost

Greater weight loss typically means more excess skin remaining.

Common areas affected

  • Abdomen — often a large hanging pannus that can extend toward the thigh
  • Upper arms — significant sagging limits clothing choices
  • Inner thighs — loose tissue causes persistent chafing and discomfort
  • Breasts — substantial volume loss and shape changes
  • Back and flanks — rolls of excess tissue, especially at the bra line
  • Buttocks — flattening and sagging following fat loss

When to consider body contouring

Timing matters. Proceeding too early can compromise results and potentially require revision surgery. A general guideline is waiting 6–12 months, or at minimum 6 months, after your bariatric procedure. This waiting period ensures:

  • Maximum weight loss has been achieved and has plateaued
  • Nutritional status has normalized — duodenal switch creates significant malabsorption, so adequate protein and replenished vitamin stores are essential for safe surgery and wound healing
  • Your body has reached a new, stable baseline
  • Medical clearance has been obtained from your bariatric surgeon and primary care physician

Your bariatric team will help confirm when you’re medically ready. Dr. Borsuk coordinates closely with bariatric surgeons throughout Ottawa and Eastern Ontario to ensure optimal timing and safety.

Body contouring options

The right procedure depends on your anatomy, goals, and areas of greatest concern. Some patients combine multiple procedures in a single surgery; others need staged surgeries spaced 4–6 months apart for safety.

Hospital-based safety

All body contouring procedures are performed at Winchester District Memorial Hospital — not a private surgical suite. For patients with nutritional considerations or medical complexity following duodenal switch, hospital-based care with an anesthetist and full emergency capability is non-negotiable for safety.

Your next step

A consultation ($180 + tax, credited toward your procedure if you proceed within 6 months) includes a complete review of your bariatric surgical history and weight-loss timeline, a comprehensive assessment of all areas of excess skin, honest discussion of realistic expectations, and a personalized surgical plan.
